About Botox

Botox is the trade name for botulinum toxin type A , a purified protein that the FDA first cleared for cosmetic use in 2002 [1]. When injected in microdoses into specific facial muscles, it temporarily blocks the nerve signal that tells those muscles to contract. The skin above the muscle relaxes. The wrinkle softens. You keep your expression, you just lose the line that comes with it.

Botox is the most-performed minimally invasive cosmetic treatment in the United States, with over 8.7 million procedures in 2022 [2]. It’s also FDA-approved for several medical conditions: chronic migraine, severe underarm sweating (hyperhidrosis), cervical dystonia, blepharospasm, strabismus, and overactive bladder [1]. At kalon Dermatology we treat both cosmetic and medical indications.

The protein itself is a naturally-occurring neurotoxin produced by Clostridium botulinum bacteria, but the medical-grade product used in injection is purified, dose-controlled, and has decades of safety data behind it [3].

We dose in microdoses placed for your face, so the muscle relaxes just enough to soften the line while your expressions stay your own. Your provider will tell you honestly when a lighter touch or no treatment is the better call.

Dr. David Biro, MD, PhD, is a board-certified dermatologist and Associate Clinical Professor of Dermatology at SUNY Downstate Health Sciences University. He earned his BA from the University of Pennsylvania, his MD from Columbia University, and his PhD from Oxford University.

How to Prepare for Your Botox Appointment

Botox is a quick procedure, and there’s not much to prepare. The few things that matter:

  • Two weeks before: stop fish-oil and vitamin E supplements. Both thin blood and increase the chance of bruising at injection sites.
  • One week before: stop NSAIDs (ibuprofen, naproxen, aspirin) unless they’re prescribed and you can’t pause. Acetaminophen (Tylenol) is fine.
  • 48 hours before: skip alcohol. Wine and beer dilate small facial vessels and bruising shows up more easily.
  • Day of: come in with clean skin, no makeup if convenient. Eat normally. Hydrate.
  • What to mention at intake: if you’ve had Botox before, bring your last unit count and treatment date. If you’ve had a cold sore at any injection site, tell us so we can pre-treat with antivirals.

If you’re getting Botox for chronic migraine or hyperhidrosis, your insurance may need a prior authorization. We can submit one for you. Please ask at booking so we can start that 2 to 3 weeks ahead.

The Botox Procedure

A typical Botox visit takes 15 to 30 minutes from check-in to walking out. Your provider cleanses the area, marks the injection points based on the muscle map taken at consultation, applies a topical numbing cream (or, for masseter Botox, a brief ice pack), and then administers a precise series of microinjections.

Cosmetic Botox Areas

Upper Face: Forehead Lines, “11s,” and Crow’s-Feet. Horizontal lines from raising your eyebrows, the glabellar lines between your brows, and the radiating lines around your eyes when you smile or squint. Dr. Biro and the team treat these with strategic placement that softens the wrinkle while preserving the brow lift you actually want.

Lower Face: Lip Flip, Chin Dimpling, Bunny Lines. Small amounts along the upper-lip border for a subtle pout. Treatment of the mentalis for “orange peel” chin texture. Treatment of the nasalis for bunny lines on the bridge of the nose.

Masseter Botox for Jawline Contour. By relaxing the masseter muscle , we soften a square jawline and reduce the bulk that comes from clenching. Typical dose is 25 to 50 units per side. Results develop gradually over 4 to 8 weeks as the muscle relaxes.

Therapeutic Botox

TMJ and Bruxism. Botox into the masseter and temporalis muscles relieves jaw tension and reduces grinding force. Patients often report less morning jaw pain within 7 to 10 days [4].

Chronic Migraine. For patients with 15+ headache days per month, FDA-approved Botox is administered across 31 fixed injection points spanning the frontalis , temporalis , occipitalis , cervical paraspinal , and trapezius muscle groups [1]. We provide insurance documentation on request.

Hyperhidrosis. For severe underarm sweating, Botox blocks the nerve signals to sweat glands. Treatment takes under 30 minutes and lasts up to 6 months [1].

Specialized Approaches

Botox for Men (“Brotox”). Men’s facial muscles are typically larger and stronger, requiring adjusted dosing and placement. The goal: rested, not “done.”

Preventative Botox (“Baby Botox”). Smaller doses for patients in their late 20s and early 30s, before deep static lines have formed.

Recovery and Aftercare

Botox has effectively no downtime. Most patients walk out, get coffee, and go back to work. The few rules that matter for the first 24 hours:

  • Don’t lie down for 4 hours after injection. Stay upright. This keeps the toxin from migrating to muscles you didn’t intend to treat [3].
  • Don’t rub or massage the treated area for 24 hours. Same reason.
  • Skip strenuous exercise, hot yoga, sauna, and steam rooms for 24 hours. Increased blood flow can accelerate diffusion.
  • Skip alcohol for the rest of the day. Aids bruise prevention.
  • Tiny bumps at the injection sites are normal and resolve in 15 to 30 minutes. Pin-point bruising can last 2 to 5 days. Arnica gel helps; we’ll send you home with a sample if you bruise easily.

If anything feels off (asymmetry, drooping eyelid, unexpected weakness in a muscle group), call us. We see all touch-up patients within 14 days at no additional charge if a small adjustment is needed.

Botox Results: Onset, Peak, and Duration

Most patients see the first signs of softening at 2 to 3 days. The result peaks at about 14 days, when all the targeted muscles are at their lowest activity. From there, the effect gradually wears off over 3 to 4 months as the nerve endings regenerate [3].

A typical maintenance schedule looks like:

  • Cosmetic Botox: every 3 to 4 months, with a small touch-up at week 2 if needed
  • Masseter Botox: every 4 to 6 months (the muscle takes longer to rebuild)
  • Migraine Botox: every 12 weeks per FDA dosing
  • Hyperhidrosis Botox: every 6 to 9 months

Patients who maintain a consistent schedule often need fewer units over time, because the muscles “learn” the lower activity pattern. Patients who let the effect fully wear off and start over each time tend to need closer to the original unit count.

Corresponding and Complementary Procedures

Botox addresses dynamic wrinkles. For the static lines that show even when your face is at rest, or for volume loss, contour, or skin texture, we may layer Botox with one or more of the following at kalon Dermatology:
